The desktop app is Medulla in its own window, with a few things a browser can't do: a floating window that stays on top of your other apps, keyboard shortcuts that work from anywhere, and Insert, which puts your note into the app you're typing in. It uses the same account as the web app and the EMR plugin.
Install on a Mac

Open the downloaded file and drag Medulla into Applications.
Open Medulla from Applications and sign in with your email.
Install on Windows

Sign in with your email.
Allow permissions
The first time you open Medulla, its Settings window opens at Permissions. Click Allow next to each one you want to use. You can come back later: open Settings from the Medulla icon in your menu bar or system tray.
Microphone records your visits. You need this one to scribe.
Accessibility (Mac only) lets Insert paste the note where you're typing.
System audio (Mac only) records the other side of a call in Zoom, Teams or Meet.
On a Mac, if you've refused a permission, its button opens the matching page in System Settings so you can turn it on there.
Keyboard shortcuts
These work while you're in any app. You can change them under Keyboard shortcuts in Settings.
Start or pause a visit: ⌘⇧M on a Mac, Ctrl+Shift+M on Windows
Finish a visit: ⌘⇧E on a Mac, Ctrl+Shift+E on Windows
Insert the note: ⌘⇧I on a Mac, Ctrl+Shift+I on Windows
Insert your note into another app
Finish the visit.
Click into the text field in the other app where the note should go.
Press the Insert shortcut, or click Insert in the floating window.
Your clipboard is put back the way it was afterwards.
Closing the app and updates
Closing the window keeps Medulla running in your menu bar or system tray, so your shortcuts keep working. To quit, click the icon there and choose Quit Medulla.
Medulla downloads updates in the background and applies them the next time it starts, so an update never interrupts a visit.
